When calling to schedule an appt please have the following:

A) Insurance Card
B) Doctor’s written order or our purple referral form with diagnosis and testing requested.
C) Please be aware that if you do not have this information with you, we may ask you to call back. This is
    done in order to schedule your appointment as accurately as possible and to avoid delays when you
    arrive for your appointment.
D) When you call for an appointment our staff will ask you the following questions:
        1) Who is your referring OB-Gyn? This is very important because we need to know who to send
            your studies and or medical records after we see you.
        2) Insurance ID Card – We need your most updated benefits in order to verify your eligibility, and
            also to obtain co-payment, deductible and co-insurance if applicable. Every insurance policy is
            different so we need to call 48 hrs in advance and obtain your policy’s benefits. If you have a
            deductible, co-payment, or co-insurance greater than $50.00 you will be notified. If your insurance
            requires an authorization, it is your responsibility to obtain and or notify your referring OB and
            provide authorization prior to your appointment. Failure to do so will result in delays and or

            click here. Please be aware that all payments will be collected at the time service is rendered. We
            accept cash and credit card only. No personal checks will be accepted.
        3) Reason why you are being referred to our facility – this is the reason why your doctor is
            referring to our facility. This is usually included in your prescription from the OB-GYN. This is
            indicated in the prescription under Diagnosis or DX. Some examples are listed below:

                a) AMA (Advanced Maternal Age)
                b) Family History
                c) Size & Dates
                d) Anatomy
                e) Screening
                f) Maternal or fetal condition

                    Please provide at time of scheduling your appointment all indications on your prescription. If
                    you have a purple prescription / referral form this will be indicated by a check in the
                    appropriate box or boxes.

        4) If you are being referred for Genetic Counseling or a Perinatal Consult with one of our physicians it
            is your responsibility to obtain all lab-work and pertinent medical records from your OB-GYN’s
            office.
        5) After you have scheduled your appointment please fill out our online forms. You will need to fill out
            the "Patient Information Form". If you are scheduled for Genetic Counseling you will also need
            to fill out the “GC Questionaire” form. Both forms are in English and Spanish. If you would
            rather print the form and bring it on the day of the appt that options is also available by
            printing the form directly and filling it out. Having these forms ready on the date of your
            appointment will make your wait time considerably less and will expedite your appointment.
        6) Having all this information ready before you call will make the process of scheduling your
            appointment much easier for yourself and our scheduling coordinator.
